How to fill it in
- Ask before the roster for those dates is built.
- Say whether it is all day or only some of the hours.
- If it is paid leave, use the leave request form instead.
- The manager replies in writing, so there is no doubt later.
Common mistakes
- Time off asked for after the roster is published, and handled like a swap
- Paid leave asked for on this form and never recorded in payroll
- A yes given in person and forgotten by the next roster
When a spreadsheet stops being enough
In Roster Flow staff mark a date or a window unavailable on their phone, and the roster flags it when a shift is placed there.
